Customer Credit Application

I am legally authorized to bind the above listed company. Should credit be granted by Proximity Malt, all decisions with respect to the amount or continuation shall be at the sole discretion of Proximity Malt and may terminate credit availability at any time. The payment terms for sales of goods or services sold on account are stated on the Proximity Malt’s invoice(s). Prepayments are due PRIOR to shipment. The failure to pay on the due date of each invoice shall deem the debt to be delinquent. In the event of a delinquency, Proximity Malt may impose a late charge of one and one half percent (1.5%) per month on the delinquent balance, until the balance is paid in full. In the event of a delinquency, all collection expenses, including collection agency fees and costs, attorney’s fees, and any court costs incurred in connection with the collection of the debt shall be due and payable by the applicant.
